 Starting your health and fitness journey with Compass Performance is straightforward!  Our onboarding process involves filling out essential forms. These help us understand your history, goals, and health needs to tailor a program specifically for you.

Initial Consultation

This form helps us get to know you better. It covers your fitness history, current abilities, and long-term goals, giving us the foundation to create a program tailored to your unique needs.

Health Questionnaire (PAR-Q)

The Physical Activity Readiness Questionnaire (PAR-Q) is essential for identifying any health conditions or risk factors that may need to be considered before starting your program. It's a quick but important step to ensure your safety during training.

Physician Approval (if required)

If your PAR-Q responses indicate any medical concerns, we may request clearance from your physician before proceeding. This ensures that training is safe and appropriate for your health status. While it may seem like an extra step, it’s a crucial part of safeguarding your wellbeing.

Waiver & Liability Release

This document outlines the scope of our training services and confirms your understanding of the terms, including our cancellation policy and assumption of risk. It’s a standard form to sign to ensure transparency and mutual understanding.
